ABSTRACT

Objective: To compare the value of three dimensional CT (3D-CT) and three-dimensional MRI (3D-MRI) in diagnosing shoulder glenoid bone loss (GBL). Methods: Totally 50 patients with shoulder instability (SI) underwent 3D-CT and 3D-MR examinations. Glenoid surface area, GBL surface area and degree of GBL measured with 3D-CT and 3D-MRI were comparatively analyzed. Digital calipers was used to measure GBL of all patients during operation, and the data were analyzed with Kappa test to observe diagnostic performance of 3D-CT and 3D-MRI. Results: The best fitting circle surface area with 3D-CT was larger than that with 3D-MRI, but there was no significant difference (P>0.05). There was no significant difference of GBL surface area nor degree of GBL acquired using two Methods: (both P>0.05). According to linear regression analysis, the regression coefficient of the best fitting circular surface area was 1.06 (R2=0.78) between 3D-CT and 3D-MRI, of GBL surface area was 1.05 (R2=0.88). GBL degree measured with 3D-CT and 3D-MRI was in good agreement with intraoperative measurement Results: (Kappa=0.82, P<0.05; Kappa=0.88, P<0.05). Conclusion: 3D-CT and 3D-MRI have the same value in diagnosis of shoulder GBL. 3D-MRI has the potential to take place of 3D-CT for providing full and non-radiative pre-operative evaluation of GBL in SI patients.

ABSTRACT

Objective To investigate the clinical effect of mesocaval shunt and selective coronary vein ligation for portal hypertension caused by cavernous transformation of the portal vein in adults.Methods The clinical data of 26 adult patients with portal hypertensive variceal bleeding caused by cavernous transformation of the portal vein treated by mesocaval shunt plus splenic artery and coronary vein ligation were analyzed retrospectively from 2001 to 2011.Results There was no operative deaths,free portal vein pressure decreased postoperatively,differences were significant before and after shunt (34.4 ± 4.7) cm H2O vs.(24.8 ±2.7) cm H2O,t =12.30,P <0.01.Postoperative complications included slight hepatic encephalopathy in 1 case,stress ulcer bleeding in 1 case,which were cured by conservative treatment.23 cases were followed up from 6 months to 4.5 years,and there was no bleeding case.Conclusious Mesocaval shunt plus selective vascular ligation for portal hypertensive variceal bleeding caused by cavernous transformation of the portal vein in adults is safe and effective.

ABSTRACT

Objective To investigate the significance of XAGE-1bmRNA expression in the blood specimens of patients with primary liver cancer, cirrhosis and benign liver diseases and in healthy individuals. Methods Venous blood specimens of patients with primary liver cancer (n= 125), cirrhosis (n= 23), benign liver diseases (n= 34) and healthy individuals (n = 41 ) were collected. XAGE-1b mRNA was detected by real-time fluorescence quantitative PCR. Results The expression levels of XAGE-1b mRNA in patients with primary liver cancer, cirrhosis, benign liver diseases and healthy in dividuals were 3.72 (0.93, 10.2) ×10-5, 0 (0, 0. 56) ×10-5, 0 (0, 0)×10-5, 0 (0, 0) ×10-5, respectively. The XAGE-1b mRNA expression in patients with primary liver cancer was obviously higher than the patients with cirrhosis, benign liver diseases and in healthy individuals. The expression levels for patients with cirrhosis was higher than patients with benign liver diseases and in healthy individuals. The expression levels for patients with benign liver diseases and healthy individuals were similar. With a optimal cut-off value of 8. 385 × 10-7 , the sensitivity, specificity, positive predictive value, and negative predictive value of XAGE-lb mRNA for diagnosing primary liver cancer were 80. 0%, 89.8%, 90.9% and 77.9% respectively. The positive rates for patients with primary liver cancer and cirrhosis were 80.0% and 30.4% respectively. Conclusion XAGE-lb mRNA can be used as a tumor marker for primary liver cancer. It contributes to the differentiation between primary liver cancer, cirrhosis and benign liver diseases.

ABSTRACT

Objective To investigate the value of obli que coronal MR images in visualization of the anterior cruciafe ligment(ACL).Methods Conventional axial,oblique sagittal and coronal MR images were performed in 30 normal knees.Then oblique coronal T 2-weighted i mages,which was parallel to ACL,based on oblique sagittal T 2 -weighted images ,were acquired.The visualization of ACL on oblique coronal,oblique sagital and coronal T 2-weighted images was analysed.Results The visualization of the whole ACL was 100%(30/30 ) on one image of the oblique coronal T 2-weighted images, 83.3%(25/30) on on e image of the oblique sagaital T 2-weighted image. On the coronal T 2-weighte d images,the ACL appeared as segmental bundle.The whole ACL could not be visua lized on one image of coronal T 2-weighted images.Conclusion The whole ACL can be showed on one image of ob lique coronal T 2-weighted image,which is helpful in evaluation of the injure o f ACL.
